2. Fill up the Application Form. - downloadable
3. Submit all documents required:
       a. Photocopy of Latest Report Card
           (for Grade School and High School)
       b. Certificate of Good Moral Character
           (for Grade School and High School)
       c. Recommendation Form- downloadable
           (for Grade School and High School)
       d. Photocopy of Birth Certificate
       e. 1x1 picture
3. Wait for the results of the Assessment.
4. Pay corresponding Fees.

ENROLLMENT PROCEDURE:
1. Fill up the Student Profile Inventory- downloadable
2. Submit required documents
        a. Original Latest Report Card
        b. 1 pc. 1x1 and 2x2 picture
        c. Immunization Record/ Clearance from Doctor
3. Pay corresponding Fees.
